REQUIRED REPORTING

Mentors are to maintain contact with their Cadet and Case Manager during the 12 month Post Residential phase and submit the the following monthly reports on time.

Each report can be filled out online and saved/downloaded. You can then upload the completed form on this page or fax to 918-824-4829

RESIDENTIAL Mentor Contacts & Community Service Documentation

As part of the Residential requirements for graduation from Thunderbird Challenge Program, the following requirements must be met: 8 hrs. of one to one time between the Mentor and Cadet. 1 hr. of community service is optional.

Post Residential Report

This report discusses the progress, or lack of progress the Cadet is making or has made.

Mentor Contact Form

Contact requirements: 4 contacts, 4 hours of contact or combination or both.

Other contacts can also be made through other forms of communication: telephone, mail,  email, Internet and even text messaging can be counted as a contact.
